ALL MVESD FAMILIES ARE BEING ASKED TO COMPLETE THE APPLICATION OR TO CHECK THE BOX TO OPT-OUT OF THE FREE/REDUCED MEAL PROGRAM
Frequently Asked Questions
Need to fill out an application for each child?
No, you can fill out one form for multiple children in the same household.
Who will know that our family submitted an application?
Our system is confidential. Even if your child qualifies and chooses to eat lunch in the cafeteria, the breakfast and lunch process is the same as a student who is paying full price.
Do I still need to fill out a form if I qualify for other state benefits?
No, if you have received a letter from MVESD Child Nutrition stating that your child qualifies for free meals.
Yes, if you are receiving state benefits but have not received a letter from MVESD Child Nutrition. Receiving state benefits does not automatically qualify your child for free meals.
Do I have to list a social security number?
If you do not have a social security number, check the box that states that you don’t have one; otherwise you only have to give the last 4 digits of your social security number which is kept strictly confidential and not shared with any organization or persons
2023-2024 School Meal Applications
2023-2024 MEAL APPLICATIONS ARE HERE!
Please read the following information before you scroll down to the bottom of this page for applications and instructions.
Welcome to Mohave Valley Elementary School District School Year 2023/2024.
Children need healthy meals to learn. Families are highly encouraged to submit a school meal application. The meal application process is still vital regardless of eligibility! Eligibilities affect many areas and programs we offer our students, such as Pandemic-Electronic Benefit Transfer (P-EBT), Federal and State funding for your school, Pay to Play sports fees, and More!
Parents and/or guardians must re-apply each school year for the free & reduced meal program.
Even if your child doesn't plan to eat school lunches, it is important to fill out the free/reduced-priced meal program application to ensure our schools and families receive the available resources.
To apply, an adult household member needs to complete an application to determine eligibility.
One application should be used to list all children in the household.
Eligibility is determined by household size and income criteria. Children from households whose income is at or below the USDA Child Nutrition Program levels are eligible. Foster children under the legal responsibility of a foster care agency or court are eligible for free meals regardless of income and should be included in your application. All children in households receiving benefits from SNAP (food stamps), the Food Distribution Program on Indian Reservations or TANF, can get free meals regardless of their income.
Please remember that the parent's responsibility is to make sure the District Office Food & Nutrition Department receives the application. Each application can take up to 10 days to process. Benefits do not begin until the date your application is approved, and you are responsible for all meals purchased before that date.
Print the application, fill it out completely with signature and bring it or mail it to the Child Nutrition Office located at 8450 S. Olive Street, Mohave Valley, AZ 86440 or return the application to your child's school. Please call Margie Poppin at 928-768-2507 if you have any questions.
Applications are also available in English and Spanish at each school's office and from the school cafeterias.
This institution is an equal opportunity provider.
